FutureU will create an HTML/CGI/Perl/Javascript software program that will allow your staff to simply put in new questions while reusing the existing data collection routines over and over again.
We will create a survey form that uses a consistent set of question structures (e.g., some Likert Scale radio buttons, some "Check All that Apply" check boxes, and one or two "Comments" boxes at the end.) A fill-in-the-blanks template interface will allow you to change the question content to suit your needs. The output is a simple delimited file that can be imported into an analysis engine, such as one of the desktop survey software packages or even Excel. This project can be completed in 6 to 12 weeks depending on design specifics.
As a possible option to an existing software analysis engine, a simple statistical analysis can be added to the underlying code for an additional fee.
Cost: $12,000, one-time license for unlimited use in perpetuity.
